It hits, then it hits three more times afterward with a massive AOE that can hit other things in the room. It's very accurate. Low damage starting but that's due to the multi-hit nature.
Testing it, I'm getting like 25 to 30 damage but attack at base level.
EDIT: I remembered wrong. It's first hit + two extra hits per attack.
